- Treatment — to provide, coordinate and manage your dental care, including sharing information with specialists and laboratories involved in your care.

- Appointment reminders and treatment alternatives — we may contact you about appointments or treatment options.

You have the right to inspect and obtain a copy of your records; request amendments; receive an accounting of certain disclosures; request restrictions on certain uses; request confidential communications; obtain a paper copy of this notice; and be notified of a breach of unsecured PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalised for filing a complaint.
