Last updated: September 3, 2026. This policy explains what information this website collects and how it is used. Our handling of your health information as a patient is governed separately by our Notice of Privacy Practices under HIPAA.
Information you give us
When you use the contact or appointment-request forms, we collect the details you enter — typically your name, phone number, email address and message — to respond to your request. Please do not include medical details in these forms; we will discuss those at your visit. Form submissions are transmitted securely and delivered to our office email; we retain them only as long as needed to respond and for our records.
Online booking
Online appointments are scheduled through our practice-management provider’s booking system, which loads within this site. Information you enter there is handled by that provider on our behalf under their own security practices and our agreement with them.
Information collected automatically
We use Google Analytics (via Google Tag Manager) and Cloudflare Web Analytics to understand how the site is used — pages visited, approximate location, device and browser type, and referring sites. Google Analytics uses cookies; you can opt out with the Google Analytics opt-out add-on. Cloudflare Web Analytics does not use cookies. We do not sell personal information.
Spam protection
Forms are protected by Cloudflare Turnstile, which may process technical information about your browser to distinguish people from bots.
Live chat
If a chat widget is present on the site, messages you send are processed by the chat provider. Please do not share medical information in chat.
Links to other sites
This site links to third-party sites (for example, insurance carriers and universities). We are not responsible for their privacy practices.
Children
This website is not directed to children under 13, and we do not knowingly collect personal information from them online.
Your choices
You can decline cookies in your browser settings, and you may ask us to delete a form submission by contacting the office.
